BACKGROUND: During the past two years, this Blog has reported on a crisis in Canadian pathology indicated by serious breakdowns in hospitals in Newfoundland, New Brunswick, Quebec, Saskatchewan, Ontario and elsewhere in the country. The purpose, beyond seeking review and reform, is to show that the wide-ranging problems with pathology in Canada were not limited to the criminal sector - and that serious errors, sometimes lethal, were being made in reading test results on living patients. In short, that there was a crisis in Canadian pathology.

"WINDSOR, Ont. - A provincial investigation looking into surgical errors and quality of pathology results at three hospitals in the Windsor, Ont., area has been completed,"
the Canadian Press story published earlier today begins under the heading, "Mistaken mastectomies report completed and handed to Ontario Health Ministry."span style="font-style:italic;

"A report by Dr. Barry McLellan, president of Sunnybrook Health Sciences Centre in Toronto, has been handed over to the Ministry of Health," the story continues.

"The report is under review and the results are expected to be released sometime next week.

A spokesperson for the minister's office calls it a “substantial and detailed report.”

The province appointed McLellan to lead the investigation in March after two women had breasts removed when they were mistakenly diagnosed with cancer."
